UPDATE (JUNE 2, 2026): Ontario Provincial Police have confirmed a second fatality following the May 29 collision on Highway 15 near Otter Lake Park in Lombardy.
Police say a six-year-old boy from Elizabethtown-Kitley who was airlifted to hospital with life-threatening injuries has since died. The child’s death comes after a 34-year-old Elizabethtown-Kitley man was pronounced dead at the scene. The investigation remains ongoing.

Child airlifted to a trauma centre after Highway 15 collision
MAY 30, 2026 — A 34-year-old man from Elizabethtown-Kitley is dead following a three-vehicle collision involving an ambulance Friday evening on Highway 15 in Lombardy.
Members of the Ontario Provincial Police Leeds County detachment responded to the collision around 5 p.m. May 29 on Highway 15 between County Road 1 and County Road 5, near Otter Lake Park.
Police said the crash involved two passenger vehicles and an ambulance that was returning to base with no patients onboard.
The driver of one of the vehicles was pronounced dead at the scene. A young child travelling in the same vehicle was airlifted to a trauma centre by Ornge with life-threatening injuries.
The two paramedics in the ambulance were taken to hospital with serious injuries. The driver and sole occupant of the third vehicle suffered minor injuries and was also transported to hospital.
The Leeds Thousand Islands Fire Service, Leeds Grenville Paramedic Service and OPP Technical Collision Investigators and reconstructionists assisted at the scene.
Highway 15 was closed for several hours during the investigation and has since reopened.
The investigation remains ongoing. Anyone with information or dashcam footage is asked to contact Leeds County OPP at 1-888-310-1122 or submit an anonymous tip through Crime Stoppers Ontario.
